On noncontrast heavily T2 WI axial images, appendix (arrow) was noted medial to the ascending colon. Appendix was dilated with fluid in the distal portion (yellow arrow) and lower signal intensity in the proximal portion suggesting pus (orange arrow). A small appendicolith (arrowhead) was also noted as a signal void dot at the appendiceal orifice.
