Fluid in the perihepatic space and both paracolic gutter (arrows) show high attenuation approximately 35 HU, suggesting hemorperitoneum. In the left adnexa, amorphous high density mass-like lesion is noted (asterisks), compatible with acute hematoma.A tiny enhancing dot showing similar attenuation with the vessels indicates extravasation.
The differential diagnosis would be the rupture of the ovary cyst or ectopic pregnancy. The urine hcg level was normal, therefore this case was diagnosed as the ovary cyst rupture.
